От толкования смысла песни соавторы воздержались (во всяком случае, подобные объяснения найти не удалось), поэтому фанатам Metallica остается лишь высказывать собственные предположения. Русскоязычная «Википедия» утверждает, что в тексте идет речь о мальчике, который состарился, делая то, что ему велели другие. Говоря обобщенно, это композиция о попытках контролировать жизни более слабых людей.Также встречается мнение, что в ней Джеймс Хэтфилд поделился тем, что ему пришлось пережить в юные годы. Возможно, приблизиться к пониманию основной идеи песни поможет его высказывание о треке “The Unforgiven III”. В интервью MTV он назвал его «продолжением той же сюжетной линии о грехе и последствиях, прощении и непрощении».
Работая над “The Unforgiven”, участники группы хотели отойти от присущего группе стиля. Так родилась идея сделать куплеты тяжелыми, а припев спокойным и мелодичным, что в корне отличалось от того, как Metallica играла предыдущие баллады. В ноябре 1992 года “The Unforgiven” вышла вторым синглом с альбома “Metallica”. Он поднялся на 15-ое место UK Singles Chart и 35-ую строчку Billboard Hot 100, а также пользовался успехом в хит-парадах многих стран на разных континентах.
Песня породила два продолжения с тем же названием и похожей тематикой: "The Unforgiven II," с альбома "ReLoad" и "The Unforgiven III" с альбома "Death Magnetic".
